A Mental Health Care Coordinator will assess needs, develop treatment plans, and effectively collaborate with individuals and their family members. The Care Coordinator will assess strengths and needs and, in collaboration with the individual, develop a person-centered recovery plan. The Care Coordinator will provide skills training, crisis prevention/management, linkage, and referral to support goal progress for children and families living with mental health challenges. This compassionate, caring, and supportive team member will contribute to the skills improvement, achievements, and overall potential success of a child and their state of wellbeing.
