The Youth PRS-Family Partner is an integral part of the Substance Use Services (SUS) team as the bridge that connects the parent/legal guardian/family with the youth program participants and the clinical staff. Under the leadership of the SUS Program Director and the PRS Team Lead, the Family Partner provides support to the parent/Legal Authorized Representative (LAR) of the youth. Access to quality family partner support can be instrumental in engaging families as active participants in the youth's substance abuse care. A family partner's personal lived experience is critical to establishing a trusting relationship and earning the respect of families who are experiencing substance use concerns with youth. Families play a pivotal role in the recovery process of young people who are experiencing substance use concerns. Family Partners can be a mediator, facilitator,or a bridge between families and agencies. They ensure each family is heard and their individual needs are being addressed and met. Through their work with parents/LARs, family partners direct impact on the youth's resilience and recovery.

Essential Duties:

- The Family Partner is responsible for serving as a formal member of the youth recovery team

- Advocacy encouraging the positive choices of the parent/LAR, promotes self-advocacy for the parents/LARs and their youth, and supports the positive vision that the parent/LAR has for their youth's recovery

- Mentoring through the transfer of knowledge, insight, experience, and encouragement, including the family partner's articulation of their own successful experience of navigating a youth service system

- Role-modeling the concepts of hope and positive parenting, advocacy, and self-care skills that will ultimately benefit the resilience and recovery of the youth

- Experience in guiding/navigating the youth-serving systems, including mental health, special education, juvenile justice, child protective services, etc.

- Connection to community resources and informal supports

- Identification of the family's natural support, strengths, and practical guidance in nurturing those relationships

- Stewardship of family voice and choice

- Listen to the families' concerns and involve them as equal partners in the planning and delivery of treatment

- Help improve communication among family members

- Be flexible in meeting the needs of the family

- Support through the facilitation of parent support groups

- No confirmed findings of person abuse/neglect

- Maintains refresher training timeframes

- Works cooperatively with the Supervisor and Team

- Communicates effectively

- Flexibility in schedule to meet families where they are.

- Willingness to travel.

A desire to use her/his personal lived experiences to help families is also necessary. Responsible for working with a team and achieving performance targets/standards. Works closely with internal and external stakeholders to provide the highest quality service and service linkage available. Attend meetings with community professionals/leadership, conduct/attend meetings with professional demeanor and attire.

Program Operational Functions:

- Interact positively and in cooperation with other members of the organization and team, including active participation in supervision activities, team meetings, and continuing education, including the Peer Support Services Team

- Maintain data tracking systems in an organized manner to comply with reporting and auditing requests in a timely fashion

- Complete all documentation and reporting requirements thoroughly and on time

You'll definitely need:

Education/Certification:

Required:

  • Identify self as a person living with a youth experiencing substance use, addiction, or a mental health diagnosis
  • Ability to communicate effectively with diverse populations
  • High School Diploma or GED
  • Current Texas Driver's License
  • Completion of Certification of Family Partner

Preferred:

Bilingual English/Spanish

We'll also want you to:

  • Maintain comprehensive documentation that meets or exceeds Department of State Health Services (DSHS) standards and Texas Health and Human Services Commission (HHSC) standards
  • Utilize Collaborative Documentation and complete all documentation of contacts within 24 hours
  • Reach out to SUS referrals within 1 business day
  • Comply with HIPAA and Protected Health Information (PHI)/Personally Identifiable Information (PII) policies/rules/laws/regulations to ensure confidentiality of all client information
  • Comply with all HCMHDDC policies and procedures, including the completion of all required training
  • Must become certified in PMAB, First Aid, and CPR within three months of employment and maintain certification without interruption thereafter.
  • Ensure timely communication within the department and with other departments and agencies via written correspondence, email, telephones, and other means to ensure project follow-through and completion
  • Value and respect every individual's recovery experience
  • Engage individuals in services in the progression of person-centered recovery goals/objectives
  • Provide face-to-face service hours to reach or exceed performance goals
  • Must be eager to promote the peer support program. This includes engaging individuals in existing services or new to services into the peer support program; this may include introductory calls (cold calls), greeting individuals waiting for services (in waiting areas), presenting information in a public setting, etc.
  • Provide timely notice to individuals served, supervisor, clinic director, and other necessary team members on changes to group schedules, appointments, etc.
  • Maintain appointment rate above 80%
  • Maintain a minimum of 25 weekly scheduled appointments
  • Client Record Audit score of 85 or above (quarterly)
