The Community Diversion Coordinator is responsible for providing services and supports to individuals living with mental illness who are involved or at risk of being involved with the criminal justice system. This includes individuals who are participating in the Mental Health Court Program or otherwise mandated to participate in mental health services. The Community Diversion Coordinator will assess the individual's strengths and needs and, in collaboration with the individual, develop a person-centered recovery plan. The Community Diversion Coordinator will provide skills training, supported housing, supported employment, psychosocial rehabilitation, assessment, linkage, and referral to support goal progress for individuals and families living with mental health challenges. The Community Diversion Coordinator is responsible for scheduling and carrying out individual and group services. This position will be required to be trained and certified in crisis intervention. Office hours are M-F 8:00 am to 5:00 pm, and extended hours one day a week; however, the position must have flexible hours to accommodate individuals who cannot meet between 8:00 am and 5:00 pm. This position will require travel, home visits, and service provision in community settings. This job is community-based and requires the ability to work autonomously with little supervision.

You will get to:

  •   Attends Court Staffing meetings and Court hearings.
  •  Works closely with court staff to ensure continuity of care.
  •  Ensures that all individuals on caseload have a current Uniform Assessment (at least 95% of all CANS / ANSA's are completed on time).
  •  Achieves an average score of 85, with no one score below 75 on the Person Centered Recovery Plan,  Quality Management audits.
  •  Completes initial interviews and enrollments for individuals new to services under the guidance and supervision of an LPHA.
  •  Provides case management services to justice-involved individuals assigned to a caseload.
  •  Schedules and delivers services that meet performance targets relevant to the position.
  •  Responsible for ensuring staff kept appointment rates of at least 80%.
  •  Must provide collaborative documentation at least 95% of the time or no later than 24 hours after delivery of service.
  •  Provides average hours as required by the Level of Care served.
  •  Will deliver services using the recommended Evidence-Based Practices recommended by HHSC.
  •  Person Centered Recovery Planning is developed in collaboration with the individual/family served and any other person or provider requested by the individual/family.
  •  Responsible for the provision of crisis intervention services as needed.
  •  Must be crisis certified within 120 days of employment.
  •  Attends work regularly and as scheduled.
  •  Works cooperatively with others and the supervisor
  •  Communicates effectively and in a timely manner with team and individuals served.
  •  Follow the agency's workplace values and principles.
  •  Maintain client confidentiality and privacy according to standards.
  •  Follow policy and procedures for protecting clients as well as reporting client abuse and neglect if needed.
  •  Protect and ensure client rights always.

You'll definitely need:

  •  Bachelor's degree from an accredited college or university in social, behavioral, or human services, and the ability to be credentialed as a Qualified Mental Health Professional.
  •  Preferred:  3-5 years of experience working in a mental health setting providing community-based services within the community.
  • Successfully complete and practice PMAB and CPR.
  • Bilingual; English/Spanish preferred.
  • Live within 30 miles of the clinic with reliable transportation
  • To maintain a valid Texas State Driver's License.
  • To have current automobile insurance and the ability to travel to individuals when necessary.

$29.88 per hour and benefits
