The Community Health Worker (CHW) is a trusted community health professional who bridges gaps between those served and local health/social services. This role focuses on improving health outcomes for underserved populations by providing culturally appropriate health education, navigating complex insurance systems like Medicaid/CHIP, and advocating for individual and community needs. CHWs help people navigate healthcare systems, manage chronic conditions, and address social determinants of health.
You will get to:
- Identify community resources and maintain an updated inventory of pertinent resources across agency catchment areas.
- Participate in community outreach events, educating our partners about our services and connecting community members to the resources needed.
- Assist individuals and families in overcoming barriers to treatment.
- Maintain accurate client records and document all activities in accordance with agency policy.
- Assist individuals and families in navigating health-related services, including completing applications for Medicaid, SNAP, and WIC.
- Attend client and treatment team meetings to provide the highest level of comprehensive care. Act as a liaison between individuals and the treatment team.
- Address Social Determinants of Health by connecting individuals to community resources for housing, food security, transportation, and legal aid.
- Provide culturally appropriate health education to individuals served.
- Provide community referrals and follow-up services.
- Bridge gaps between behavioral health and substance use treatments.
- Coordinate with the Crisis Continuity team, Mental Health Court, as well as other specialty programs, to ensure individuals receive timely and appropriate connection to services.
