Intensive Case Manager (Hays County I-35 Corridor)

This position will engage high-need youth and their families in developing, implementing, and monitoring an intensive, individualized plan of care. This position helps the youth and families to identify a support team who will collaboratively design a plan based upon family needs, strengths, and goals. The Wraparound Facilitator will support the team as they implement the plan and monitor its success. This position will require some early evenings. The position must have flexible scheduling and travel is required.

You will get to:

  • Coordinates and facilitates monthly Wraparound team meetings
  • Provides leadership in the development, implementation and evaluation of individualized plans using the phases and activities of Wraparound
  • Implements the YES Waiver program and completes all required documents within identified time frames
  • Develop trust and mutual respect with youth and their teams
  • Assists teams to identify needs and create a shared vision
  • Assists families in identifying, recruiting and engaging natural supports
  • Enables teams to take ownership of the Wraparound process
  • Supports clients in identifying their strengths and building upon them
  • Develops the problem-solving skills and self-efficacy of the youth and team members
  • Identifies barriers to full participation in the Wraparound process and develops strategies to overcome them
  • Ensures the Wraparound process is completed with high fidelity to the model and tracks progress towards goals.
  • Provides support and advocacy for Wraparound teams
  • Prepares summary documents and final reports
  • Ensures that all required confidentiality and other compliance documents are current for each youth
  • Educates families, providers and the community about the Wraparound process
  • Contributes to and supports a positive, team-oriented work environment
  • Utilizes Collaborative Documentation and 95% of all documentation entered within 24 hours
  • Turn Service Request Forms (if applicable) within 24 hours with a denial rate of less than 5%
  • Provide at least 75% Person Centered Time each month or an average each quarter
  • Promotes and demonstrates Hill Country MHDD Centers mission, vision and values through both behavior and job performance on a day-to-day basis
  • Effectively interacts and communicates with Hill Country MHDD Center staff/ persons in service and their families/customers and partnerships
  • Knowledge of or experience with Wraparound Model
  • Strong written and verbal communication
  • Proficient in computer applications such as Microsoft Word or Excel
  • Organized and effective multitasker

You'll definitely need:

  • Bachelor's degree from an accredited four (4) year college or university with a major in social, behavioral or human services.
  • One year of experience working with children and youth with serious emotional disturbance.

Preferred

  • Three years of experience working with children and youth with serious emotional disturbance
  • Bilingual (Spanish/English)
  • NWIC Model - Wraparound Training
  • A valid Texas State Driver's License.
  • Current automobile insurance and the ability to travel when necessary.
