The Continuity of Care Jail Care Coordinator is a dedicated individual in the Forensic Services Division, embedded within the Comal County Jail. In this role, you will provide crisis intervention services as needed, along with providing planning and case management for inmates with mental health and/or substance use conditions who are eligible for pre- and post-release from the jail. This position would also act as a liaison between the County Jail, HCMHDD Centers, and local community resources to ensure continuity of care for individuals with the intention of reducing recidivism rates related to mental health and substance use conditions. Duties include coordinating behavioral and physical health needs while in jail and upon release from jail; providing behavioral health consultation and follow-up with primary care and behavioral health care patients. The delivery of services will be based on the needs of the community and individuals experiencing mental health needs. The Jail Care Coordinator will assess specific needs to be able to develop and implement Person Centered Recovery plans that support individuals with mental illness/substance use in learning skills to reach their specified service outcomes and to attain/maintain individual recovery.

Essential Duties and Responsibilities:

  • Assesses all individuals with positive jail screening and/or referred consumers from jail staff to determine appropriate referrals needed or if a doctor visit is recommended. Completes all needs assessments for incarcerated individuals within 3 business days.
  • Ensures that all individuals have a current Uniform Assessment prior to release.
  • Must provide collaborative documentation at least 95% of the time or no later than 24 hours after delivery of service.
  • Will deliver services using the recommended Evidence-Based Practice recommended by HHSC.
  • Person Centered Recovery Planning is developed in collaboration with the individual/family served, and any other person or provider requested by the individual/family.
  • Maintain grant data collection.
  • Respond to individuals in mental health crisis by providing immediate emotional support and active listening.
  • Establish a supportive and non-judgmental relationship with individuals in crisis, demonstrating empathy and understanding.

You'll definitely need:

  • A bachelor's degree from an accredited college or university in psychology, sociology, behavioral, or human services, with the ability to credential as a Qualified Mental Health Professional
  • A valid Texas State Driver's License and a safe, inspected, and roadworthy vehicle
  • Current automobile insurance and the ability to travel to individuals when necessary.
  • Must meet the criteria that indicate a commitment to safe and responsible driving, as defined by the Texas Risk Management Council.?

We'll also want you to:

  • Preferred: 3-5 years of experience working in a mental health setting providing community-based services within the community.
  • Ability to work as a team member, in a diverse environment, interfacing with both internal and external customers.
  • Ability to work in a rapid-paced environment in an organized and professional manner.
  • Working knowledge of Mental Health issues, services, and treatment, as well as knowledge of the criminal justice system.
  • Ability to self-monitor production and quality with minimal assistance.
  • Basic knowledge and familiarity with data systems, including business software such as Microsoft Office.
  • Bilingual: English/Spanish preferred.
