The Next Step Program Care Coordinator (QMHP-CS) is responsible for providing services and support to adults with mental health conditions who are without consistent and safe housing in reaching their vision of recovery. The Care Coordinator will assess the individual's strengths and needs, and in collaboration with the individual and treatment team, develop a Person-Centered Recovery Plan (PCRP) with interventions designed to meet their individual outcomes and goals. The Care Coordinator will provide skills training, psychosocial rehabilitation, linkage and referral; crisis prevention/management, Supported Housing, and Supported Employment services. This position is responsible for timely and accurate documentation, as well as monthly data analysis and reporting as required through the Next Step grant. The Care Coordinator will work directly with the LPHA, shelter staff, and community partners in ensuring comprehensive and effective communication within the treatment team. The Care Coordinator will attend community events and meetings that best support education, outreach, and continuity of care to unhoused adults in Comal County. The Care Coordinator will obtain/maintain certification to provide crisis intervention services to individuals as needed. The Care Coordinator is responsible for scheduling and carrying out individual and group rehabilitation services and skills training. The Care Coordinator is responsible for administrative duties as needed to maintain individual authorization to receive services in accordance with TAC and Hill Country MHDD policy. Typical hours are 8:00 am - 5:00 pm, but flexibility in scheduling is required to best accommodate individuals served.

You will get to:

  • Ensures that all individuals on caseload have a current Uniform Assessment (at least 95% of all CANS / ANSA's are completed on time).
  • Achieves an average score of 80, with no one score below 75 on the "Person Centered Recovery Plan" Quality Management audits.
  • Must provide collaborative documentation at least 95% of the time or no later than 24 hours after delivery of service.
  • Provides at least 75% Person Centered Time each month.
  • Will deliver services using the recommended Evidence-Based Practice recommended by HHSC.
  • Person Centered Recovery Planning is developed in collaboration with the individual/family served and any other person or provider requested by the individual/family.
  • Schedules and delivers services within the authorized Level of Care in accordance with Texas Resiliency and Recovery standards
  • Achieves 3% of Supported Housing and Supported Employment targets

You'll definitely need:

  • A bachelor's degree from an accredited college or university in psychology, sociology, behavioral or human services, or a related degree.
  • A valid Texas State Driver's License.
  • Current automobile insurance and the ability to travel to individuals when necessary.
  • Preferred: 3-5 years of experience working in a mental health setting providing community-based services within the community.
  • Preferred: 2-3 years of experience working with individuals experiencing homelessness.
  • Experience working within a team setting.
  • Bilingual: English/Spanish preferred.
  • Live within 30 miles of the clinic with reliable transportation

We can pay you:

$21.96 - $22.99 per hour and benefits
