The Regional Director of Service Coordination
has the primary responsibility of supervising Service Coordination in assigned
areas. Reporting to the Director of IDD Authority Services, duties include
managing and supervising Service Coordinators and Office Coordinators at each
assigned program site, hiring new staff, and ensuring that staff are
appropriately trained to perform their essential job functions. This position
is responsible for ensuring that all individuals served receive outcome-based
services and support. Must work collaboratively with LIDDA Management Team
members. Must have knowledge of IDD eligibility criteria, service delivery
options, state and federal requirements for service delivery, and LIDDA
operations, as well as person-centered practices.

- Model person-centered practices in all aspects of job performance.
- Coach staff in positive skill-building techniques for
improved job performance (e.g., problem solving)
- Effectively manage conflict, promote change and growth, and
inspire the development of all stakeholders (individuals in services,
community, and staff)
- Provide flexibility in supervisory strategies based upon the
needs and preferences of staff, to include modeling in the field,
shadowing, and other in-person supervision.
- Develop and implement strategies that will maximize synergies
among program areas.
- Develop strategies for integration into currently existing systems.
- Interfaces with referral sources, providing information and
status reports as required by referring agencies/programs.
- Represent HCMHDDC Service Coordination in learning
opportunities as needed.
- React to change productively and handle other tasks as assigned.
Team
Management and Development:
- Provide professional leadership in organizing, administering,
supervising, and evaluating the effective delivery of Service Coordination
- Develop and implement a system to evaluate the skill, experience,
and professional development needs of all staff.
- Work with staff to develop objective performance measurements
across all sites, to ensure consistent, high-quality evaluation, and goal
setting for all parts of the program.
- Instill a sense of accountability among team members by
modeling tight oversight of individual and organizational performance standards.
- Actively participates in a Continuous Quality Improvement
(CQI) philosophy.
- Recruit, hire, and ensure training and orientation of all
program staff.
- Develop an atmosphere of respect, interest, and enthusiasm in
the services provided.
- Recognize and encourage outstanding performance by people
within the program.
- Conduct regular staff meetings.
- Inform the proper authority when attendance issues are present or non-compliance is evidenced.
Program
Operational Management:
- Must be available to provide service coordination and other
duties as assigned when staff vacancies arise.
- Ensures that all individuals served within assigned areas
receive face-to-face contact according to the frequency included on their
person-directed plan.
- Appropriately make program assignments to Service
Coordinators and ensure that services are initiated in a timely manner.
- Ensure that documentation standards are consistently met by
all staff supervised.
- Ensure that all supervised staff meet the required timeframes
for data reporting and documentation.
- Ensures that all performance contract requirements are met.
- Maintains a proactive knowledge of requirements and
guidelines set by the state and federal governments.
You'll
definitely need:
- Bachelor's
or Advanced Degree with a major in a social, behavioral, or human service field, including, but not limited to, psychology, social work, medicine, nursing,
rehabilitation, counseling, sociology, human development, gerontology,
educational psychology, education, and criminal justice.
- Three
years of experience providing Service Coordination or case management to
individuals with IDD (Required)
- One
year of experience in management and supervision of staff (preferred)
- To maintain a valid Texas State Driver's
License.
- To have current automobile insurance and the
ability to travel to work locations frequently.

- Maintains comprehensive documentation that meets or exceeds
Texas Health and Human Services Commission standards.
- Utilizes Collaborative Documentation, and 95% of all
documentation is entered within 24 hours.
- Comply with HIPAA and PHI to ensure confidentiality of all
client information.
- Work closely with the Human Resources team to provide the
highest quality work environment and staff support possible.
- Work closely with Quality Management/Utilization Management
to ensure an open, ongoing CQI structure.
- Be proactive in community outreach and education to
strengthen relationships with community stakeholders.
- Successfully complete and practice PMAB, CPR.
- Requires flexibility in schedule and travel.
- Other duties as assigned.
