The Texas Correctional Office on Offenders with Medical or Mental Impairments (TCOOMMI) Adult Probation Intensive Case Manager (ICM) is responsible for providing person-centered intensive case management services to adults diagnosed with severe and persistent mental health disorders who are currently on the specialized Mental Health Initiative probation caseload in Hays County, Texas.

The position reports to the TCOOMMI Program Director and requires the development and maintenance of effective, professional working relationships with Hays County Adult Probation Officers and other community service providers. This is a Monday through Friday, non-exempt, field-based position with flexible hours.

TheTCOOMMIICMregularlymeetswithindividuals intheirhomes, community, probation/parole, court, and at Hill Country MHDD clinicstoprovide case management, skills training, and psychosocial rehabilitativeservices fora caseload of20-25 individuals. Services are intended to stabilize mental health symptoms, reduce the risk of recidivism, increase the individual's awareness of and participation with community and natural supports, develop self-advocacy, and assist with their ability to participate in independent behavioral health services. Staff expectation is to provide 3.5 billable service hours per individual on the TCOOMMI caseload every month. A high level of detailed electronic documentation is required, and adherence to company and state guidelines is expected. The person hired in this position should be knowledgeable about the Texas Department of Criminal Justice, TCOOMMI Program Guidelines and Processes, PGP-01.02.

The person in this position must possess a passion and aptitude for working with individuals who have chronic and severe mental health needs and are involved in the criminal justice system. Knowledge of psychotherapeutic medications, intensive case management functions, substance use treatment, crisis services, community resources, and evidence-based practices in the mental health and criminal justice systems is required. This position necessitates innovative thinking, patience, resiliency, skills in advocacy, service coordination, mental health assessment and evaluation, collaborative person-centered recovery planning, and dual-diagnosis treatment. The person in the position will effectively work independently and on a team, prioritize needs during crisis situations, engage in regular meetings and training opportunities, balance self-care, be a delight to work with, and perform other duties as assigned with a positive attitude.

You will get to:

  •  Promote and maintain a trauma-informed, person-centered environment for all clientele and staff.
  •   Ensures services provided are in accordance with the TCOOMMI Program Guidelines and Processes for ICM.
  •   Ensure everyone receives no less than 3.5 monthly hours of behavioral health care services in accordance with the TDCJ TCOOMMIPGPO1.02.Regularly communicate with the Program Director with feedback, concerns, and solutions to meet expectations.
  •  Initiate contact with individuals served and/or the Legal Authorized Representative to assess needs, provide collaborative services, person-centered recovery planning, facilitate referrals and linkages to appropriate resources, monitor service effectiveness, provide advocacy, facilitate crisis intervention or prevention services. These services need to be provided in a timely manner. The services are in accordance with the HHSC and TCOOMMI contract/guidelines.
  •  Initiate and reciprocate coordination and collaboration with members of the Recovery Team and/or other Intensive Recovery Teams, Program Director, Center Directors, peer specialists, and other Hill Country employees/departments. Initiate and reciprocate coordination and collaboration with community members, as appropriate, to better provide quality services, including support service delivery.
  • Coordinate intake and assessment of individuals referred to the TCOOMMI program.
  •  Ensure that clinical documents and tools necessary for state contracts are completed electronically in a timely manner to assure services are clinically appropriate and documented. This includes completion of, but is not limited to, uniform assessment (ANSA), Person-Centered Recovery Plans, Demographics, Diagnosis, and Financial Assessments.
  •  Ensure all individuals have a current Person-Centered Recovery Plan(PCRP) that identifies intensive case management as an intervention.
  •  Identify the psychosocial needs of the individual and facilitate their attendance at related services.
  •  Identify and coordinate the individual's access to all available therapeutic and behavioral health services, including inter- and intra-agency resources. Serve as an advocate for the individual.
  •  Provide individual service in the areas of activities of daily living, coping skills, problem-solving, stress and anger management, social and recreational skills, psychiatric symptom management, effective communication, and psychosocial functioning recovery.
  • Ensure all service activities are clearly documented in the electronic health record system, and documentation indicates interventions performed, progress made, and the relation of services to recovery plan goals.
  • Ensure that all documentation is entered into electronic health records within 24 hours of the time the service(s) were provided.
  •  Provide purposeful, thoughtful transitional and/or discharge services with approval and consultation of the Program Director and/or with otherwise assigned clinical staff, as appropriate.
  •  Develop and maintain strong, professional, and effective collaborative working relationships with probation and other community agencies.

  •  Document all services to include kept appointments, no-shows, cancellations by individuals, and self.

  •  Participate in regular clinical supervision with the Program Director and/or designee.
  •  Participate in professional training, center meetings, and staff development training as required.
  •  Protects client confidentiality by being knowledgeable of and adhering to HIPAA regulations.
  •  Adhere to all Hill Country MHDD policies and procedures.
  •  Maintain CANS/ANSA certification.
  • Travel and driving company or personal vehicle as required for service delivery and professional development.
  • Contribute to a positive, collaborative work environment. Cooperative, cordial, and honest interactions with all employees are observed.

You'll definitely need:

  • Bachelor's degree in social sciences from an accredited university.
  • Preferred: Master's degree or college-level courses in Criminal Justice or Social Sciences from an accredited university
  • Previous experience working in the TCOOMMI program, behavioral health, criminal justice, and/or case management 
  • QMHP certification 
  • Must become certified in PMAB, First Aid, and CPR within three months of employment and maintain certification without interruption thereafter.
  • Bilingual (English/Spanish) (preferred).
  • A valid Texas State Driver's License
  • Current automobile insurance and the ability to travel when necessary.
