The Coordinated Specialty Care (CSC) Care Coordinator for First Episode Psychosis is an early intensive intervention program for adolescents and young adults (ages 15-30) who have been recently diagnosed with a mental health condition which includes psychosis. The program goals include reducing the duration of untreated symptoms, and improving outcomes such as the ability to work, go to school, and live independently and have enjoyable relationships. The role of Care Coordinator is to help individuals overcome barriers to their recovery using structured behavioral interventions aimed at learning new skills and supporting behavior change. This position will provide coordination, monitoring and rehabilitative services for adults meeting First Episode Psychosis eligibility. The Care Coordinator will assist eligible individuals to establish meaningful personal recovery goals through the person-centered treatment planning process. This position also includes outreach and recruitment activities. Office hours are 8:00 am ? 5:00 pm; however, this position must have flexible hours to accommodate individuals who cannot meet between 8 and 5. This position requires flexibility in schedule to meet non-traditional work hours to include potential for weekend hours.

You will get to:

• Provide intensive treatment that supports the individuals' own goals for recovery and shared decision making
• Consistently meets the agency performance standards for the Care Coordinator position
• Be flexible and consistent in providing services, you must provide average hours as required by the Level of Care served
• Provide engagement/initial assessment, ongoing intervention/monitoring/reassessment of need, and transfer of care/planning for future needs/services
• Provide relevant skills training to improve daily life. These areas of focus include medication adherence, medical care, supported education/employment, family support, behavioral interventions (including social and coping skills training and substance use treatment), housing/income, trauma-informed care, and safety planning/suicide prevention
• Strengths-Based and Person-Centered Care
• Convey empathy, hope and optimism, provide treatment choices for individuals, meet individuals where they are, and support individuals' own strength and hopes for change
• Ensure that all individuals on case load have a current uniform assessment and are authorized for service
• Collaborate with the treatment team and with service providers to be a liaison for effective care

?  Achieves an average score of 80, with no one score below 75 on the ?Person Centered Recovery Plan? Quality Management audits

?  Must provide collaborative documentation at least 95% of the time or no later than 24 hours after delivery of service

• Person Centered Recovery Planning is developed in collaboration with the individual/family served, completed in a timely manner, and updated consistently to align with current needs and goals

You'll definitely need:

• Bachelor's degree from an accredited college or university in social, behavioral, or human services with the ability to be credentialed as a Qualified Mental Health Professional (QMHP-CS)
• Preferred: 3-5 years of experience working in a mental health setting providing community-based services within the community.
• Maintain valid Texas state Driver's License and auto insurance.
• Complete and maintain both CPR certification and Prevention and Management of Aggressive Behavior training within the first 3 months of employment.
• Bilingual: English/Spanish preferred.

• Document in the Electronic Health Records System and assure that records are up to date and in conformance with agency standards
• Attends all relevant team meetings 
• Maintains compliance with all Department training requirements
