The Texas Correctional Office on Offenders with Medical or Mental Impairments (TCOOMMI) Parole/Probation Intensive Case Manager (ICM) provides person-centered intensive case management services to adults diagnosed with severe and persistent mental health disorders on the Special Needs Offender Parole caseload and the Mental Health Initiative Probation Caseload in assigned counties. The position requires development and maintenance of effective, professional working relationships with County Adult Parole & Probation Officers and other community providers. The TCOOMMI ICM must possess a passion and aptitude for working with individuals who have chronic and severe mental health needs and are involved in the criminal justice system, to stabilize mental health symptoms, reduce risk of recidivism, increase the individual's awareness of and participation with community and natural supports, develop self-advocacy, and assist with their ability to participate in independent behavioral health services. This is a Monday through Friday, non-exempt, field-based position with flexible hours.

You will get to: 

•  Meet with individuals in their homes, community, parole/probation departments, court, and HCMHDD clinics for case management, skills training, and psychosocial rehabilitative services for a caseload of 20-25 individuals, providing at a minimum 3.5 billable service hours per individual every month per TCOOMMI PGP and a maximum of 5.87 billable service hours per the Texas Administrative Code.
•  Maintain a high level of detailed electronic documentation and adherence to company and state guidelines.
•  Possess innovative thinking, patience, resiliency, skills in advocacy, service coordination, mental health assessment and evaluation, collaborative person-centered recovery planning, and dual-diagnosis treatment.
•  Work independently and on a team, prioritize needs during crisis situations, engage in regular meetings and training, balance self-care, and perform other job duties as assigned with a positive attitude.
•  Promote and maintain a Trauma-Informed, Person-Centered environment for all clientele and staff.
•  Ensure services provided are in accordance with the TCOOMMI PGP for ICMS. Regularly communicate with the Program Director with feedback, concerns, and solutions to meeting expectations.
•  Initiate timely contact with individuals and/or their Legal Authorized Representative (LAR) to assess needs, support person-centered recovery planning, coordinate referrals, and monitor service effectiveness. Ensure all services, including advocacy and crisis intervention, are delivered in accordance with Health and Human Services and TCOOMMI guidelines.
•  Coordinate and collaborate with Recovery Team members, internal departments, and community partners to support the delivery of high-quality, person-centered services. Facilitate intake and assessment processes for individuals referred to the TCOOMMI program while maintaining effective cross-team communication.
•  Ensure all clinical documentation required for state contracts is completed accurately and electronically in a timely manner, including ANSA, PCRP, demographics, diagnosis, and financial assessments. Maintain current Person-Centered Recovery Plans for all individuals, ensuring intensive case management is identified as an active intervention.
•  Identify and assess the individual's psychosocial needs while coordinating access to appropriate therapeutic and behavioral health services across internal and external resources. Advocate for the individual by providing support in daily living skills, coping and problem-solving, emotional regulation, communication, and overall psychosocial recovery.
•  Ensure all service activities are accurately documented, detailing interventions, progress, and alignment with recovery plan goals, with collaborative documentation achieved at least 95% of the time, including all kept appointments, cancellations, and no-shows.
•  Meet the agencies' current productivity standard for the position.
•  Provide purposeful, thoughtful transitional and/or discharge services with approval and consultation of the Program Director and/or with otherwise assigned clinical staff.
•  Develop and maintain strong, professional, and effective collaborative working relationships with parole, probation, community agencies, and treatment team members.
•  Travel as required for service delivery and professional development.

You'll definitely need:

•  A bachelor's degree from an accredited college or university in social sciences with the ability to credential as a Qualified Mental Health Professional.
•  Preferred: A master's degree from an accredited college or university in social sciences with the ability to credential as a Qualified Mental Health Professional.
•  Maintain a valid Texas state Driver's License and auto insurance.
•  Knowledgeable about the Texas Department of Criminal Justice, TCOOMMI Program Guidelines and Processes, PGP-01.02.
•  Two years of experience working with adults with serious mental illness involved in the criminal justice system, with strong knowledge of psychotherapeutic medications, intensive case management, substance use treatment, crisis services, community resources, and evidence-based mental health and justice practices.
•  Complete and maintain ANSA Certification.
•  Complete and maintain both CPR certification and Prevention and Management of Aggressive Behavior training within the first 3 months of employment.
