Yes Waiver Intensive Case Manager/Wraparound Facilitator (Hays County MH Center)

The YES Waiver Intensive Case Manager/Wraparound Facilitator supports high-need youth and their families through intensive care coordination, Wraparound facilitation, and individualized service planning. This position works closely with families and supports teams to identify goals, monitor progress, and promote long-term success. Flexible scheduling, evening hours, and travel required.
You will get to:
- Build strong relationships with youth, families, and teams while facilitating engagement, natural support, and shared ownership of the Wraparound process.
- Support strength-based planning using the phases and activities of Wraparound by developing problem-solving skills, promoting self-efficacy, addressing participation barriers, and advocating for youth and families.
- Coordinate and facilitate Wraparound team meetings, lead individualized service planning, and ensure high-fidelity implementation and progress toward goals and a shared vision.
- Implement the YES Waiver program, completing required documents within program time limits.
- Educate families and providers on the Wraparound process while collaborating with internal and external stakeholders to support coordinated care and successful outcomes.
- Prepare summary documents and final reports.
- Utilizes Collaborative Documentation, and 95% of all documentation is entered within 24 hours.
- Turn in Service Request Forms (if applicable) within 24 hours with a denial rate of less than 5%.
- Provide at least 75% Person Centered Time each month or an average each quarter, behavior, and job performance on a day-to-day basis.
- Must be certified in PMAB, First Aid, and CPR within three months of employment and maintain certifications without interruption thereafter.
You'll definitely need:
- Bachelor's degree from an accredited college or university in social, behavioral, or human services, with the ability to be credentialed as a Qualified Mental Health Professional (QMHP-CS)
- Minimum one year of experience working with children and youth with serious emotional disturbance.
- Knowledge of or experience with the Wraparound Model, NWIC Model
- Maintain a valid Texas state Driver's License and auto insurance.
