The Assisted Outpatient Treatment Care Coordinator is responsible for providing services and supports to individuals living with mental illness who are participating in the Hays County Assisted Outpatient Treatment Program (AOT). The Assisted Outpatient Treatment Care Coordinator will assess the individual's strengths and needs and, in collaboration with the individual, develop a person-centered recovery plan. The Assisted Outpatient Treatment Care Coordinator will provide skills training, supported housing, supported employment, psychosocial rehabilitation, assessment, linkage, and referral to support goal progress for individuals' families living with mental health challenges; crisis prevention/management, Supported Housing, and Supported Employment services. The Assisted Outpatient Treatment Care Coordinator is responsible for scheduling and carrying out individual and group services. This position will be required to be trained and certified in crisis intervention. Office hours are M-F 8:00 am - 5:00 pm, and extended hours one day a week; however, the position must have flexible hours to accommodate individuals who cannot meet between 8:00 am and 5:00 pm. This position will require travel, home visits, and service provision in community settings.
You will get to:
- Ensures that all individuals on caseload have a current Uniform Assessment (at least 95% of all CANS / ANSA's are completed on time).
- Achieves an average score of 85, with no one score below 75 on the Person Centered Recovery Plan and Quality Management audits.
- Schedules and delivers services that meet performance targets relevant to the position.
- Responsible for ensuring staff kept appointment rates of at least 80%.
- Must provide collaborative documentation at least 95% of the time or no later than 24 hours after delivery of service.
- Provides average hours as required by the Level of Care served.
- Provides at least 75% Person Centered Time each month.
- Will deliver services using the recommended Evidence-Based Practice recommended by HHSC.
- Person Centered Recovery Planning is developed in collaboration with the individual/family served and any other person or provider requested by the individual/family.
- Responsible for the provision of crisis intervention services as needed.
- Must be crisis certified within 120 days of employment.
