The Mobile Unit Care Coordinator serves as a Qualified Mental Health Professional - Community Services (QMHP-CS) responsible for operating a mobile clinic unit to deliver behavioral health services across rural and underserved areas within Hill Country MHDD Center's (HCMHDDC) catchment area. This position combines direct service provision with logistical oversight, including driving the mobile unit, ensuring equipment setup, maintaining a safe and functional mobile care environment, and delivering individualized services in accordance with the Texas Resiliency and Recovery (TRR) model and agency standards.

This role involves providing person-centered, trauma-informed care through screenings, assessments, case management, crisis intervention, skills training, and coordination of services for individuals experiencing mental health challenges. The Mobile Unit Care Coordinator works independently in the field and collaborates with multidisciplinary teams to address community needs, increase access to care, and support recovery and resilience in diverse populations.

Essential Duties and Responsibilities:

· Ensures that all individuals on caseload have a current Uniform Assessment (at least 95% of all CANS / ANSA's are

completed on time)

· Must provide collaborative documentation at least 95% of the time or no later than 24 hours after delivery of service.

· Will deliver services using the recommended Evidence-Based Practice recommended by HHSC.

· Develops and monitors person-centered recovery plans in collaboration with individuals served and their support

systems.

· Operates and maintains the mobile clinic vehicle, including driving to scheduled locations, ensuring proper setup and shutdown of equipment, and maintaining cleanliness and functionality.

· Provides crisis intervention services, including screening, assessment, and placement as needed.

· Coordinates with internal teams and external partners to connect individuals to appropriate services, including

medical, housing, and substance use treatment.

· Ensures compliance with safety protocols, HIPAA regulations, and ethical standards in all field-based operations.

· Participates in community outreach, events, and partnerships to support engagement and reduce barriers to care.

· Maintains flexible work hours that meet the needs of communities served.

· Participate in disaster response activities when incident command is mobilized.

You'll definitely need:

  • A bachelor's degree from an accredited college or university in psychology, sociology, behavioral or human services, or a related degree.
  • A valid Texas State Driver's License.
  • Current automobile insurance and the ability to travel to individuals when necessary.
  • Preferred: 3-5 years of experience working in a mental health setting providing community-based services within the community.
  • Experience working within a team setting.
  • Bilingual; English/Spanish preferred.
  • Live within 30 miles of the clinic with reliable transportation

· Maintains CANS/ANSA certification. 

- Successfully complete and practice PMAB, CPR.

    · Travel as required for service delivery and professional development.

    · Driving company or personal vehicle as required for service delivery and professional development.

    · Performs other duties as assigned by the Manager/Director.
